ST AGNES CYO BASKETBALL SIGN UPS (Grades 3-8)
VOLLEYBALL / FOOTBALL UNIFORM TURN IN
To participate in Basketball, you must attend sign-ups on Mon, 11/02 or Tues, 11/03, at 6:30pm, in the St. Agnes School gym. ***Please comply with this process, as it ensures an accurate athlete count as we plan teams & coaches accordingly.
Practices may begin week of Mon, Nov 9th.
A parent or guardian must accompany the student athlete to registration & pay the $92 fee. All CYO athletes must be registered on-line prior to St. Agnes registration. Only register on-line one time for the entire school year at www.cyojwa.org.
*Mandatory on-line Parent Release Forms & Archdiocese Registration:
To complete this mandatory form on-line, go to www.cyojwa.org. Click on CYO on-line registration, then follow the prompts (you will need a credit card ($20) to complete the online process.
In addition, students & parents still need to attend sign-ups to pay registration fees. If you are unable to obtain access to a computer (or credit card) to complete the on-line process, accommodations can be made to have access to school’s computer lab & credit card at sign-ups.
Uniform turn-in is also these same dates. A separate table will be available for uniform turn in only people. ***Please turn in your uniforms on these dates, as it assists greatly in keeping track of the St Agnes CYO uniforms !!!
